Responsibilities: - Provide case management services to patients, including assessment, planning, implementation, and evaluation of patient care - Collaborate with healthcare team members to develop and implement individualized care plans - Conduct utilization management activities to ensure appropriate use of resources and services - Monitor and assess patient progress, making adjustments to care plans as needed - Document patient interactions, interventions, and outcomes accurately and timely - Coordinate and facilitate communication between patients, families, and healthcare providers - Advocate for patients' needs and ensure they receive high-quality care Qualifications: - Valid Registered Nurse (RN) license - Minimum of 2 years of nursing experience in a clinical setting - Experience in utilization management or case management preferred - Strong knowledge of vital signs monitoring, diagnostic evaluations, and patient care techniques - Familiarity with ER procedures and protocols - Ability to work effectively with diverse patient populations, including pediatrics and behavioral health patients - Excellent communication and interpersonal skills - Strong organizational and time management abilities Note: This job description is not intended to be all-inclusive.
